A simple, membership‑based model that strengthens the patient‑provider relationship. Care is delivered without the constraints of insurance billing.

Primary Care (DPC)
Membership (Ages 18–64)
$70 / month
- Annual physicals; vision screening
 - Diabetic onsite testing (HbA1c)
 - Routine lab monitoring
 - Referrals to specialists & diagnostics (e.g., mammogram, colonoscopy)
 - Chronic condition management (HTN, DM, HLD, COPD)
 - Routine medication refills
 - Acute/Sick visit (1 per month during business hours; additional visits billed per pricing to the right)
 - Access via text / portal / email (responses within 24h, M–F)
 
VIP Membership
Ages 18–64
$85 / month ($105 value)
- Everything in DPC
 - Acute Care: two‑visit limit included
 - Telehealth extended / after‑hours
 - After‑hours / weekend / holiday urgent access
 - Medical weight loss
 
Acute Visits
$40 (included with DPC as noted)
- Incision & drainage, wound care, simple sutures
 - Urine pregnancy testing
 - Rapid testing: strep, flu, COVID
 - COPD exacerbation / bronchitis
 - Ear wax removal
 - Rash / allergy
 - Nausea / vomiting / abdominal pain

VIP Extended Hours
Telehealth only
Mon–Fri: 7:00a–8:00a & 5:00p–10:00p • Sat–Sun: 9:00a–3:00p
Policies
- Visit cancellations within 24 hours may incur a $25 fee.
 - Membership cancellation requires 30 days’ notice.
 - If a visit requires medication, the medication cost is not included.
 - We are not currently accepting insurance. We are working to add insurance in the near future.
 - Hours may change based on patient needs and provider availability.
 
How We Care
Telehealth or home visits during business hours are included in membership. VIP adds extended telehealth access and one extra acute visit per month.
